What actually started working

Third step: Car equals awesome things. I put his favorite old blanket on the back seat. Got him a special KONG toy that he only gets in the car, stuffed with peanut butter. The idea was to make him associate the car with super good stuff, not just scary motion or boring errands.
Fourth step: Consistency and ignoring (sometimes). This was tough. We started taking short car trips just for fun, ending at a park or a trail, so it wasn't always the vet or groomer. And, if I knew he'd been potty, wasn't thirsty, wasn't hurt, and he started that habitual whine? I had to ignore it. Turned the radio up a tiny bit, focused on driving. The instant he was quiet, even for a second, I'd say "Good boy!" quietly. Rewarding the silence, not the noise.
Fifth step: Safety gear helped too. Got him a proper doggy seatbelt harness. He seemed calmer being securely clipped in, not sliding around on turns. For longer trips now, we use a travel crate again, but one I made super comfy inside, and he's way more used to it after all the other steps.
Now? He's like 90% better. Seriously. He just hops in, lies down, maybe watches the world go by. Super long trips, like hours, he might get a bit restless and give a little whimper, but it's nothing like before.
